The erythrocyte sedimentation rate is a time-tested marker of inflammation. A raised ESR signals that something inflammatory is going on — infection, arthritis, TB or other chronic conditions — and prompts further targeted testing.
It is almost always interpreted together with CBC and CRP rather than alone.
